TERMS AND CONDITIONS OF YOUR MEMBERSHIP
Savings are based on the provider's regular fees or national average fees for the same service or product. Your actual savings will vary depending upon your location and the specific services or products purchased. The Eckerd HealthCare Discount Plan may not be used in conjunction with any other discount plan or program. All listed or quoted Eckerd HealthCare Discount Plan prices are current prices only and are subject to change without notice. From time to time certain providers may offer certain products or services to the general public at promotional prices which are lower than the Eckerd HealthCare Discount Plan price. In such an event, Eckerd HealthCare Discount Plan members will be charged the lower price.

Eckerd HealthCare Discount Plan is a discount membership program only and is not insurance. Although Eckerd HealthCare Discount Plan undertakes to screen participating providers to ensure appropriate credentials and qualifications to perform professional services and provide related products, Eckerd HealthCare Discount Plan does not otherwise guarantee, nor is it responsible for the quality of care or products provided by any provider.

Eckerd HealthCare Discount Plan providers are subject to change without notice. Plan benefits may vary in some states. The Eckerd HealthCare Discount Plan program may be discontinued or modified at anytime. Should you choose to cancel your membership as a result of a modification, the liability of Eckerd HealthCare Discount Plan will be limited to refund of any membership fees paid for the period subsequent to the modification

Unless cancelled by you or Eckerd HealthCare Discount Plan, your membership will automatically renew each billing period. Each renewal of your membership shall constitute a separate and distinct membership contract for the applicable period.
